MediLogz is a hybrid Health Passport & Referral Tracking System developed under Smile Health Initiatives, the flagship health program of African Worthy Cause Initiatives (AWCI). It bridges the gap between digital health innovation and on-the-ground care by giving patients, caregivers, and health workers a lifelong health record that is accessible both online and offline.
MediLogz ensures that no patient’s history is lost when they move between clinics, districts, or even countries thus strengthening continuity of care across Africa.
The Problem We Address
Many patients in Africa move between health facilities without carrying past records.
Referral slips get lost or forgotten, leading to duplication of tests, treatment gaps, and higher costs.
Lack of data sharing and follow-up systems causes missed appointments, incomplete treatments, and poor outcomes.
Our Solution
MediLogz offers a smart, integrated health record system:
✅ Digital Health Passport – A secure mobile dashboard for patients, caregivers, and health workers.
✅ Plastic NFC Health Card – Works offline as a scannable passport for hospitals without internet.
✅ Referral Tracking – Patients receive digital + paper referral slips that can be tracked across facilities.
✅ Emergency Info Access – Blood type, allergies, and emergency contacts are visible instantly.
✅ Integration Ready – Connects with hospital systems, DHIS2, and MediVoice for follow-ups and reminders.
How It Works
Patient registers and receives a MediLogz NFC card linked to their profile.
Health workers update records digitally (or offline, syncing later).
Referrals are tracked so the next facility already knows the patient’s history.
Caregivers can access summaries, treatment history, and reminders.
Ministries & hospitals get aggregated data dashboards for better planning.
Impact Goals
Reduce loss-to-follow-up by 40% in referred patients.
Cut unnecessary repeat tests and costs by 30%.
Strengthen immunization and chronic care tracking for children and adults.
Empower caregivers with lifelong, portable health records.
